Diabetes mellitus is a pressing global health challenge, yet nearly 44% of adults living with diabetes globally remain completely undiagnosed.1 According to data from the International Diabetes Federation (IDF), approximately 537 million adults (ages 20–79) live with diabetes worldwide, representing over 10.5% of the world’s adult population.1 

Projections indicate that by 2045, approximately 783 million adults (1 in 8 people globally) will live with the condition, marking a 46% increase.2 This sharp trajectory underscores the critical need for early clinical detection, lifestyle modifications, and reliable access to continuous care. Early identification is vital. Unmanaged hyperglycemia paves the way for microvascular and macrovascular complications, including irreversible blindness (diabetic retinopathy), cardiovascular disease, peripheral nerve damage (neuropathy), and chronic kidney disease leading to kidney failure.3,4

What is diabetes?

Diabetes mellitus is a progressive metabolic disorder characterized by chronic hyperglycemia (high blood sugar). It develops when the pancreas fails to produce sufficient amounts of the hormone insulin, or when the body’s cells exhibit a blunted response to the insulin being produced, rendering it ineffective.5,6 

Blood Sugar (Glucose) and Insulin Dynamics 

To understand diabetes, it helps to understand how the body processes energy:

  • Glucose Intake: The body converts dietary carbohydrates (starches and complex sugars) into glucose, which enters the bloodstream to serve as your body's primary molecular fuel.7
  • Insulin Production: In response to rising blood sugar, the beta cells inside the pancreas secrete insulin.8
  • The "Key" Analogy: Insulin acts as a biochemical key. It binds to cellular receptors, signaling the cells to absorb glucose from the blood to convert it into usable cellular energy.8 In people with diabetes, this pathway breaks down. Glucose is trapped in the bloodstream because insulin is either entirely missing or unable to open the cellular doors, resulting in systemic high blood sugar levels.

Main Types of Diabetes Mellitus

  1. Type 1 Diabetes (T1D): A chronic autoimmune disease where the body’s immune system mistakenly attacks and permanently destroys insulin-producing beta cells in the pancreas.[5] It accounts for roughly 5–10% of all diabetes cases globally.
    1. The Sugar Myth: Eating excess sugar does not cause Type 1 diabetes. Its triggers involve a complex interplay of genetic susceptibility, environmental factors, and potential viral exposures.[12]Insulin Dependence: People with T1D produce zero natural insulin and require daily, life-sustaining exogenous insulin therapy via multiple daily injections (MDI) or an automated insulin pump to live.12
    2. Demographics: Previously called "juvenile diabetes," modern data indicates that over 62% of all new Type 1 diagnoses occur in adults aged 20 or older.13 Globally, nearly 8.75 million individuals live with Type 1 diabetes.13
  2. Type 2 Diabetes (T2D): The most common form, accounting for over 90% of all diagnoses globally.14 It is driven by insulin resistance—a state where cells ignore insulin's signals—compounded by the pancreas gradually losing its ability to secrete enough extra insulin to override the resistance.6,14 Contributors: Global escalation is primarily driven by societal urbanization, an aging population, physical inactivity, and rising obesity rates.16 On-Set: Unlike T1D, Type 2 diabetes develops slowly, often remaining silent for years. Roughly 20–30% of individuals already show early signs of nerve or vascular damage by the time they receive a clinical diagnosis.17,18
  3. Latent Autoimmune Diabetes in Adults (LADA): Often called "Type 1.5 diabetes," LADA shares clinical features of both Type 1 and Type 2 diabetes.22,23 It manifests in adults over age 30 as a slowly progressing autoimmune attack on beta cells. Because it develops gradually, up to 10% of individuals initially misdiagnosed with Type 2 diabetes actually have LADA, eventually requiring full insulin therapy as beta cell function decreases.24
  4. Gestational Diabetes Mellitus (GDM): Characterized by hyperglycemia that is first recognized during the second or third trimester of pregnancy, affecting approximately 21.1 million live births (nearly 17%) globally.2 It occurs when placental hormones cause severe insulin resistance that the pancreas cannot overcome. While GDM typically resolves immediately after childbirth, it significantly increases the long-term risk of developing Type 2 diabetes later in life for both the birth mother and the child.15,26

Signs and Symptoms of Diabetes

While Type 1 diabetes symptoms typically appear rapidly and acutely, Type 2 and LADA symptoms develop subtly over time. Common symptoms across all types include:3

  • Polyuria (frequent, excessive urination)
  • Polydipsia (unquenchable thirst and persistent dry mouth)
  • Extreme fatigue and lack of energyBlurred vision (caused by fluid shifts swelling the lenses of the eyes)Slow-healing cuts, scrapes, or bruises
  • Recurrent skin, urinary tract, or yeast infections
  • Unexplained weight loss (highly prominent in Type 1)
  • Paresthesia (tingling, pain, or numbness in the hands and feet)

While some risk factors are unmodifiable, others can be altered through targeted preventative care.

Ethnic Disparities and Susceptibility

Clinical surveillance shows that systemic health inequities, genetics, and socioeconomic barriers drive sharp disparities in diabetes prevalence.14,15 In the United States, for example, the age-adjusted prevalence of diagnosed diabetes highlights a disproportionate burden on minority communities:

  • American Indian / Alaska Native populations: 13.6%
  • Non-Hispanic Black / African American populations: 12.1%
  • Hispanic / Latino populations: 11.7%
  • Asian American populations: 9.2% (with South Asian subgroups experiencing elevated risk at significantly lower BMI thresholds)
  • Non-Hispanic White populations: 6.9%

Diabetes Diagnosis and Treatment

Diabetes management is highly individualized and depends directly on the underlying physiological cause.

Clinical diagnosis is universally confirmed via specific blood markers:

  • Fasting Plasma Glucose (FPG): >= 126 mg/dL (>= 7.0 mmol/L)
  • Hemoglobin A1C (HbA1c): >= 6.5%
  • Oral Glucose Tolerance Test (OGTT): >= 200 mg/dL (>= 11.1 mmol/L) after a 2-hour glucose load

Contemporary Management Strategies

Type 1 Treatment: Requires lifelong, intensive exogenous insulin administration via multiple daily injections (MDI) or an Automated Insulin Delivery (AID) system that communicates directly with a Continuous Glucose Monitor (CGM) to auto-adjust dosing.12

Type 2 Treatment: Focuses initially on structured nutrition (like the Diabetes Plate Method) and regular aerobic exercise to reduce insulin resistance.19 If lifestyle modifications are insufficient, first-line oral medications like metformin, or advanced injectable therapies like GLP-1 receptor agonists and SGLT2 inhibitors, are introduced to protect cardiovascular and renal health.19,21

Monitoring Protocols: Traditional finger-stick blood glucose meters (BGMs) remain vital, but care has rapidly shifted toward Continuous Glucose Monitors (CGMs). These wearable sensors stream real-time data and trend arrows to smartphones, allowing patients to target a Time in Range (TIR) above 70% of the day to avoid complications.27 Diabetes is a highly manageable condition. Receiving a diagnosis can feel overwhelming, but by taking a proactive approach, adopting small lifestyle shifts, and working hand-in-hand with a multidisciplinary healthcare team, individuals with all forms of diabetes can live healthy, long, and fulfilling lives.
